Register as Physician
Register below as an Individual Provider.
If you are the Medical Director of a Group Practice and want to register your Medical Group, with multiple physicians and providers, then you will do so once registered as an individual physician and logged in to your account by clicking link to Create New Physician Group. This will enable athletes/parents to send sports physical forms to the Group, allowing all physicians and providers of your Group to view and fill forms.
